After traveling and meeting so many of you this year, one thing became clear:
We need more space to connect, share, and learn from each other.
Off stage, off screen, and in real life.
That’s why we’re launching Longevity Docs Tables
This new series of private, physician-hosted roundtables blends the intimacy of a dinner with the depth of a clinical discussion.
The first Table kicks off at my home in New York City with my dear friend Dr. Frank Lipman on how to build a concierge longevity practice.
From there, we’ll bring them to cities across the country; creating space for small, curated groups of physicians to sit down, trade insights, and build the future of longevity medicine together.

Dr. Naisohn Arfai launches his longevity practice in Miami
Last week, I launched my own solo longevity practice in Miami: HAYA Precision. HAYA stands for Healthier As You Age. I saw my first two patients last week, and I feel more energized than I have in years.
After two decades in Emergency Medicine, most recently as Director of Quality for the Emergency Department at Jackson Memorial Hospital in Miami and as AOA Faculty, I’ve seen firsthand what it looks like when disease has already taken its toll.
That’s why being part of this new field is so inspiring to me. Longevity medicine is about shifting the focus from reactive care to true prevention and health optimization.
The Longevity Docs Summit and the community represent the kind of paradigm shift that so many of us in medicine have been waiting for. The spirit of innovation, and the collaboration across disciplines (medicine, business, industry) it’s a breath of fresh air. It gives me real hope for the future of healthcare, which has been stuck in an old model for far too long.
The Longevity Docs Certification program has helped me build my practice on a very solid, scientifically grounded foundation, bringing the best insights from the frontiers of medicine.
There’s something about the momentum in this movement that makes me excited to be a doctor again. I’m here for it, and I plan to stay involved for many decades to come. Honestly, I feel 20 years younger.

Less Labor, More Longevity
This Labor Day weekend is a reminder of why we built this community: to make the practice of longevity medicine sustainable, credible, and collaborative. While most of the world celebrates labor by taking a break, we’re reflecting on the future of physician work and how Longevity Docs is helping doctors work smarter, not harder.
Labor Day was born out of the late 19th-century labor movement in the United States. It honored the contributions of workers, fought for better conditions, and redefined what fair labor looked like in a modernizing world. Fast forward to today, the same ethos applies to medicine: physicians are working longer hours, burning out, and navigating broken systems. Longevity Docs exists to help physicians build practices that honor their expertise, time, and wellbeing.

GLP-1 watch: hair loss, auto-refills, and dose strategy
Notable uptick in telogen shedding (esp. tirzepatide), likely from rapid weight loss + protein/micronutrient gaps.
What docs do: protein targets, iron/multi, platelet topical, red-light, Nutrafol/Xtresse; consider micro-dosing during aggressive loss.
Multiple reports of LillyDirect auto-refills despite “0 refills” in notes, teams flagging for patient education.
Exosomes & biologics compliance
Members shared fresh FDA warning letters
Strong consensus do not inject exosomes for derm/MSK outside appropriate pathways
Many market products are EV-rich/conditioned media rather than bona fide exosomes.
Vendor diligence and website language are critical.
Brain fog / POTS toolbox
Methylene blue (4–25 mg) might show signal, Modafinil (50–200 mg) as a short-term “reboot” while you treat root causes; LDN, GLP-1 (select cases), and a mito stack (1-MNA, SS-31, carnitine, creatine, ETC support).
Niagen IV vs NAD drips
Some claim quicker infusions, fewer sides reported vs NAD
But members push for how we measure impact and tie it to outcomes (not just serum nudges). If we can’t measure it, we can’t scale it.

Each week, I highlight a few studies that caught our eyes and could shape the future of longevity medicine.
Gum Health & Vascular Aging
A 2-year RCT (European Heart Journal) in 135 patients showed intensive periodontal treatment reduced carotid intima-media thickness and improved endothelial function versus standard care. Inflammation markers also dropped. Treating gum disease may directly slow vascular aging, making dental health a critical lever in cardiovascular longevity care.
👉 Question for Longevity Docs: If treating gum disease slows vascular aging, should dental health be part of every cardiovascular longevity conversation?
Plaque vs Cholesterol
A JAMA cohort of 23,143 symptomatic adults showed that even patients with severely elevated LDL-C had low ASCVD and death rates if both CAC and noncalcified plaque were absent. Risk climbed in proportion to plaque burden, not cholesterol level alone.
👉 Question for Longevity Docs: Soft plaque and CAC remain debated, should they become the new standard in cardiometabolic assessment or at least be discussed with every patient?
Ultra-Processed Food & Fertility
A controlled crossover trial in Cell Metabolism shows that ultra-processed diets (even without extra calories) increase weight, worsen LDL:HDL ratios, lower key metabolic and reproductive hormones (incl. FSH), and impair sperm motility, with pollutant shifts (phthalates ↑).
👉 Question for Longevity Docs: If food processing is a biomarker of aging and fertility risk, should we be debating food quality (not just calories) with every patient?
I comb through the mainstream headlines, handpick the most relevant longevity stories.
The New Rules Of Longevity: Why Women Need A Completely Different Approach To Aging
Aging well isn’t about expensive biohacks, it’s about fundamentals like hormones, muscle, bone, and brain health, especially for women navigating menopause. Experts stress that longevity shouldn’t be gamified into a competition but embraced as a privilege, with proactive choices driving real healthspan gains. Women’s Health
The Future Of Wellness: Top Trends In Longevity & Medical Tourism
According to the Global Wellness Institute, the desire to live healthier, longer lives is fueling the trillion-dollar wellness and medical tourism industry. Research from Booking.com echoes this, showing that travelers increasingly prioritize trips that enhance their overall wellbeing. Forbes
Mark Cuban’s war on America’s $5 trillion healthcare machine: ‘They can’t react as quickly’
Most drug prices today are set by pharmacy benefit managers (PBMs), or third parties that manage prescription drug programs. Cuban said drug prices are opaque by design. That’s exactly why he launched Cost Plus Drugs in 2022 — to pull back the curtain on drug pricing, bring down costs to the average consumer, and disrupt the traditional pharmacy industry. TechCrunch

Lura Health Advances Mouth-Based Health Monitoring Technology
Lura Health has partnered with Japan’s SCO Group to deploy sensor technology that monitors health through the mouth. The collaboration will advance Lura’s microsensor technology through clinical validation and expand applications globally.
Lura Health develops wearable microsensors that attach to dental appliances or teeth to collect real-time health data including pH levels and other markers in saliva. The technology aims to monitor whole-body health through the mouth.
OpenAI and Retro Biosciences achieve 50x increase in expressing stem cell reprogramming markers.
