Issue 64: The front page of longevity medicine - curated by doctors, for doctors.
Hey Doc,
It’s 4:52 am in NYC. My family is asleep, and I’m still replaying what happened on stage at the Vibrant Longevity Summit Austin. What would you say if someone read you your own eulogy while you were still alive?
That’s exactly what I did with Peter Attia. The moment I’ll never forget was seeing Peter laugh, reflect, and take in his own eulogy with warmth and humility, with the front row full of Longevity Docs watching.
We went deep on the biomarkers shaping the future of heart health, the interventions he believes hold the most promise, and the framework he uses to protect his emotional health and relationships.
This week reminded me why we built Longevity Docs: to create moments that go beyond science, moments that shape the culture of longevity medicine.
Healthy sunday!

I Read Peter Attia His Eulogy
Two days ago, I was on stage with Dr. Peter Attia at the Vibrant Longevity Summit in Austin. I opened with a question: “You’ve said what matters in longevity is the eulogy, not the résumé. What do you mean by that?”
Peter replied: “At your funeral, no one talks about how hard you worked, how much money you made, or how many companies you built…”
I said: “I actually wrote a eulogy for you. Can i read it.”
“ Dear Peter, we’re in the year 2093. You’ve reached 120, just like you predicted. You even won several Centenarian Decathlons, now an Olympic sport thanks to you.
I remember first meeting you in the hallway of a longevity conference in Vegas, back when casinos and drinking were still a thing. Then again in Austin, at Barton Creek, at a startup conference called Vibrant Summit that grew into one of the largest health systems in the world. And yes, this was back when humans still met face to face.
You were the father of Medicine 3.0. Today we’re at Medicine 9.0: nanobots deliver therapies in real time. No doctor needed anymore
Few ever met you in person, but everyone knew your voice. The Drive became one of the greatest archives in longevity medicine. Outlive 7: The Science and Art of Wisdom is sold out everywhere. Early Medical is finally covered by insurance…about time.
Zone 2 Day is a national holiday. Lp(a) testing is mandatory at birth. And David the protein bar company? Somehow became a cod fish farming company. Sorry about that one.
Being your patient was like trying to get on the La Ferrari waitlist: nearly impossible. Yet you gave so much freely: the science, the unsponsored podcasts, the transparency. Some disagreed with you, but no one doubted, the stubborn fairness and the impact you had on so many doctors
Your hugs were never warm: maybe because of those shoulder prosthesis but your voice warmed the world of longevity. You protected your time fiercely, with Nick and Lacey as your guardians. And you showed Jill and the kids your deepest love when you went to the Bridge To Recovery.
And yes, your calcium score Peter always stayed under six. Your uncle Francis would be proud.
So my dear Peter, thank you for your science, your drive, your impact, and above all, your love of longevity.”
We then discussed cardiovascular diseases and precision testing, including ApoB, Lp(a), sterols, SDMA/ADMA, homocysteine, and ceramides (all part of the Vibrant Cardio Zoomer) and how these markers drive heart disease prevention and management.

We finished the 60-minute conversation with a surprise Egg Boxing session. Discover the video👇
Every week, the Longevity Docs WhatsApp group feels like a front-row seat to the future of medicine. Here’s what had doctors buzzing:
EMR Wars: Which systems are actually working for physicians in 2025?
Longevity Docs compared experiences with EMRs like MD-HQ, Elation, Athena, eClinicalWorks, Vibrant, Practice Better, Charm, Modmed, and Practice Fusion. Frustrations centered on cost, clunky UX, and lack of customization. AI scribes are becoming standard: FreedAI, Nabla, Sunoh, and Vibrant’s built-in tools were all discussed.
Key Takeaways for Doctors:
No perfect EMR exists: most systems have trade-offs; lean toward the one that fits your practice model (cash vs insurance, size, specialty).
AI scribes are here to stay: saving time on charting and documentation is now expected, not optional.
Hidden costs matter: watch for nickel-and-diming (texts, e-Rx, % revenue cuts). Factor this into long-term sustainability.
Community feedback is invaluable: peer reviews help avoid costly mistakes when switching systems.
Metabolic Tech: PNOE, VO₂ Master, Cosmed - which devices deliver?
Longevity Docs compared VO₂ Master, PNOE, Calibre, and Cosmed systems for RMR/REE, VO₂max, and substrate utilization testing. Cost, report accuracy, mobility, and reliability were the biggest concerns. Some noted glitches (Cosmed reports not uploading), while others liked PNOE’s mobility despite high costs.
Key Takeaways for Doctors:
PNOE and VO₂ Master are popular but have trade-offs: mobility vs accuracy.
Cosmed is considered industry standard but less user-friendly and glitch-prone.
Cost vs function is a key decision: PNOE/Calibre are expensive; Cosmed is cheaper but clunky.
Future-proofing matters: new devices (e.g., VO₂ Master with CO₂ sensors in 2026) may shift the landscape.
Not All Stem Cells Are Equal: The Fight Over Safety, Science, and Regulation
A complex case of an 18-year-old with escalating autoimmunity sparked debate on MSCs: bone marrow vs umbilical vs amniotic sources. Arguments centered on immune privilege, FDA approvals (BM MS
Cs in GvHD), safety, and GMP standards. Opinions varied, but consensus was that product quality and regulation matter most.
Key Takeaways for Doctors:
Not all MSCs are the same: source, donor quality, and GMP production determine safety and efficacy.
Regulatory clarity is lacking: only bone marrow MSCs are FDA-approved (for GvHD). Everything else is experimental.
Autoimmunity use cases are emerging: early trials show promise in Crohn’s, diabetes, arthritis, but risks remain.
Education needed: call for a dedicated MSC event highlights demand for physician-to-physician clarity.

Each week, I highlight a few studies that caught our eyes and could shape the future of longevity medicine.
The multiomics blueprint of the individual with the most extreme lifespan
A multiomics deep dive into the world’s oldest living person reveals that her extreme lifespan is accompanied by features like telomere attrition and clonal hematopoiesis, yet she remains largely disease-free. Key protective traits include rare beneficial genetic variants, low inflammatory signatures, a rejuvenated microbiome, and a younger-appearing epigenome. This work identifies promising biomarkers and biological pathways for healthy aging, but broader studies are needed to validate intervention strategies. Cell Reports Medicine
Biomarkers of female reproductive aging in gerotherapeutic clinical trials
Therefore, this article discusses the available (FSH, AMH, Inhibin, antral follicle count) as well as the emerging biomarkers (estradiol, progesterone, LH, Sirtuin- 1, microRNAs, menstrual blood markers, epigenetic markers, ovarian stiffness, vaginal microbiome markers, survey information) of female reproductive aging and a protocol for evaluating the impact of gerotherapeutics in clinical trials. Scientists reverse stroke damage with stem cells (in Mice)
Scientists in Zurich have shown that stem cell transplants can reverse stroke damage by regenerating neurons, restoring motor functions, and even repairing blood vessels. The breakthrough not only healed mice with stroke-related impairments but also suggested that treatments could soon be adapted for humans, marking a hopeful step toward tackling one of the world’s most devastating conditions.
