Progesterone: Oral vs. Transdermal vs. Vaginal?
Yesterday at our Mastermind, one of the most talked about moments was a powerhouse panel featuring Dr. Felice Gersh, Dr. Amy Killen, Dr. Elizabeth Yurth, moderated by Dr. Joseph Raffaele, diving into the pros and cons of oral, transdermal, and vaginal progesterone.
As Dr. Steven Murphy put it in the chat:
“Now the problem is, I’m conflicted about oral versus vaginal.”
Dr. Catherine Johnson shared:
“It’s so personalized. Some women much prefer vaginal progesterone as they experience lessened pelvic congestion and fewer mood issues. But most prefer the anxiolytic and pro-somnolent effects of oral micronized progesterone (OMP).”
She emphasized the need to track hepatic markers, SHBG, and serum hormone levels, and added a best practice reminder:
“Since there’s less data on endometrial protection with vaginal or transdermal forms, we recommend screening endometrial thickness with TVUS every 6–12 months.”
Dr. Felice Gersh added fuel to the fire:
“I’m going to write more on vaginal progesterone vs oral - there’s really a lot of data! And also… static vs. cyclic HRT. Re-examining what is currently taken for granted keeps us on our toes. I love these discussions with smart and passionate docs!”
Dr. Elizabeh Yurth concluded:
“Doing something is always better than doing nothing”

Amanda Khan, MD
Born and raised on Manhattan’s Upper East Side, Dr. Amanda Kahn brings a deep understanding of the fast-evolving landscape of health, beauty, and wellness. A magna cum laude graduate of Columbia College in ecology and environmental biology, she earned her MD from Weill Cornell Medical College.
Her early research in genetics at Columbia Medical Center laid the foundation for her expertise in personalized medicine, aging, and metabolic health. As a Board-Certified internist, she has cared for thousands of patients, specializing in weight management, obesity, and related conditions like diabetes, hypertension, and osteoporosis. During the COVID pandemic, she worked on the front lines in the hospital’s heart failure unit, providing unwavering care to critically ill heart transplant patients.

Every Doctor Should be a Longevity Doctor
As a heart surgeon, I was often the doctor you hoped never to need . If you were in my operating room it means prevention was no longer an option. My father, a physician specializing in Chinese medicine and acupuncture, taught me about preventive care very early on. At 21, in 2001, I launched a foundation focused on operating on children in Africa, India, Cambodia, and Haiti so they could live longer, healthier lives.
In 2020, everything changed. When my wife contracted COVID-19, I realized the future of medicine lies in prevention. This led me to found a digital clinic dedicated to combating preventable age-related diseases, immersing myself in longevity research and new tech.
However connecting with like-minded doctors was the hardest part. I started a WhatsApp group with a dozen doctors passionate about longevity. We shared publications, new tech tools, and discussed case studies. It quickly grew into something bigger, and I realized we lacked a structured platform to learn, exchange insights, and shape the future of evidence-based longevity medicine.
From this need, Longevity Docs was born. Today, we unite 300 physicians across 50 countries with a shared goal: to democratize longevity medicine. Imagine a world where physicians have access to evidence-based practices, collaborate with experts, and conduct research together - in real-time, anywhere in the world.
I firmly believe that a decentralized, collective intelligence of physicians is the key to extending human healthspan. Together, we can create a legacy where medicine helps people live better.
