Payment Processing Pain: High-Risk, High-Friction for Longevity Clinics
Behind the scenes of innovation lies an unsexy but critical challenge: getting paid. This week, frustration boiled over around Square, Stripe, and high-risk merchant classifications affecting clinics that sell supplements, peptides, or memberships.
“Stripe held our money for 8 weeks due to a clerical error. Never again.” – Dr. MG
“Square is flagging us for offering peptides. It’s insane.” – Dr. NP
“Anything over $4k? I just ask for a wire. Not giving 3% to a middleman.” – Dr. SM
Business Question: Can longevity clinics build reliable, compliant payment infrastructure without bleeding margin or facing shutdowns?
Bottom Line: Clinics operating in longevity, hormone therapy, or supplements often get flagged as “high-risk.” Building a resilient payment stack or partnering with processors who understand the space is now essential.
What to Watch: Expect a rise in niche payment services tailored for high-trust, recurring care models. Clinics that solve this quietly will win big.
MRI Protocols Under Fire: Are Full-Body MRI Helping or Hurting?
Following Function Health’s acquisition of Ezra, physicians reignited debate on the utility and quality of full-body MRI in longevity care. While some clinics see value in integrating advanced imaging, many voiced concerns over cost, protocol standardization, and real-world yield.
“Ezra was the worst in every aspect.” – Dr. RP
“Not as bad as Prenuvo. Their MRI protocols were substandard.” – Dr. LP
“They use the same protocol across body parts. That’s not precision imaging.” – Dr. JS
“Minimal yield. We used to only scan when we saw circulating DNA abnormalities.” – Dr. FC
Clinical Question: Is full-body MRI an effective screening tool for preventive longevity care or is it an expensive, low-yield offering best reserved for select cases?
Bottom Line: While advanced imaging feels futuristic, the clinical value depends on tailored protocols and appropriate patient selection. One-size-fits-all scanning undermines precision care and may create false reassurance or unnecessary follow-up.
What to Watch: As more longevity clinics consider bundling diagnostics like MRI, expect new standards to emerge on when, how, and why to scan and how to counsel patients on results.

Replacement as an aging intervention
A new Nature Aging review (Lore et al., 2025) explores how replacement therapies—from tissue engineering to brain–computer interfaces—are emerging as practical interventions for age-related decline. Muscle Power Versus Strength as a Predictor of Mortality in Middle-Aged and Older Men and Women
In a 21-year study of 3,889 adults aged 46–75, low muscle power was linked to a 6x higher risk of death, outperforming strength as a mortality predictor.
Muscle power (force × velocity) was significantly more predictive of survival than grip strength in both men and women.
