Galleri promises to detect multiple cancers—but new evidence casts doubt on this much hyped blood test
The bad press builds. I wonder how many patients who paid >1k for this test feel reading this. Has anyone talked to their rep lately?
Yeah, I think tests based on circulating tumor cells, cell fragments, or cf-DNA are a dead-end. Its a numbers game. In order to detect any sort of signal from these in 2 or 3 small tubes of blood, there needs to be an existing tumor "burden" sufficient enough to generate such signals.
More promising tests seem to be looking for protein or mRNA or stem-cell signals that are NOT dependent on tumor burden, rather look for other patterns that indicate a "tumorgenic" environment.
There is a Canadian company called Stage Zero Life Science (SZLS) that developed a multi-cancer detection test based on mRNA signals. Its a promising test, but mRNA has proven to be too unstable for large-scale roll out. My favorite so far is the India-based Tzar Labs.
I remember talking to them about three years ago. Really cool stuff they're doing. I'd love to see it in the US
Agreed. I think I’ve posted here before about how the Gallieri has unimpressive test performance. I like the datar tests the best. But gallieri has far and a way best mindshare due to superior marketing and distribution. That stuff always carries the day over science unfortunately
I have a patient who uses HBOT at home and was asking if it makes a difference taking antioxidant boosting supplements around the HBOT?
Most will not need exogenous anti-inflammatory. In fact, it’s probably detrimental to achieving the desired effect. HBOT works by increasing oxidative stress via increase in cellular respiration which will increase superoxide. This allows for the desired DNA repair, stem cell release, and decrease in inflammatory cytokines.
When HBOT is administered properly (dose matters) the body will ramp up its own antioxidants including superoxide dismutase. It’s all about balancing oxidative stress. Unfortunately, when exposed to external oxidative stress such as smoke, toxins etc. – we do not have a mechanism to ramp up our internal antioxidants – hence the requirements for exogenous antioxidants.
Agree with A., the short answer is not recommended to take extra antioxidants around HBOT unless antioxidant deficiencies are present. Here is a great paper on what A. described by Dom D’Agostino and colleagues.
In essence oxidation is the engine of how HBOT works under many circumstances but not all. There’s also oxygen delivery and pressure ongoing too during treatment. However, in our sicker patients who cannot mount their own antioxidant response, we can potentially use them. My sense is that for many of these patients, it would initially be better to NOT do hbot until they were supported rather than going directly into this chamber unless there was an acute need! This gets me in trouble in the hbot community sometimes but I don’t mind :)
